The practice area covers Armadale, Blackridge and Westfield (a detailed map can be found in reception).
Please be aware that when a patient is not registered with a practice for any reason, the period of non-registration may invalidate specific policies, e.g. travel insurance policy.
A registration form for each family member is available at our Reception Desk. We require to see two forms of identification – one with current address and the other photographic.
Newly Registered Patients
All medical services are available to you from the day you register. On registering you will be given the opportunity to make an appointment with the Practice Nurse for a new patient medical. This allows us to find out about any health problems you may have without having to wait for your medical records to arrive from your previous practice.
Change of Name, Address or Telephone
Please inform the surgery as soon as possible if you change your name, address or telephone number.
